Made-to-Order Request Form

Please submit one form for each item that you want.
    Please indicate exact Item# found on the Made-to-Order product page.

    Be specific in the modifications you want from the original item : names, shapes, sprinkles, glitters, gloss, etc.
    We will respond to you within 24-48 hours.

    We serve the right to refuse modifications if they will alter the base price of the item. We will inform you of such decision and you may opt to stick to the original structure with minor modifications or pay additional costs.

    Our email will include our confirmation, shipping quote, production time, delivery date and details for non-applicable modifications for your item.
